Brewery Tour Waiver

Acknowledgment of Risk

I understand that participating in a brewery tour involves potential risks, including but not limited to:

• Walking on wet, uneven, or slippery surfaces.
• Exposure to hot equipment, chemicals, and moving machinery.
• Possible injury from falls, burns, or contact with equipment.
• Consumption of alcoholic beverages and the associated risks.

I acknowledge that these risks could result in personal injury, illness, property damage, or in rare cases, serious harm. ____________________________________________________________________________________

Assumption of Responsibility

By signing this form, I agree to:

• Follow all safety instructions given by brewery staff.
• Remain with the group at all times during the tour.
• Refrain from touching or interfering with any brewery equipment.
• Consume alcohol responsibly and only in designated areas.

I further acknowledge that if I am under the influence of alcohol or drugs, I may be refused participation in the tour. ____________________________________________________________________________________

Release of Liability

In consideration of being permitted to participate in the brewery tour, I, on behalf of myself, my heirs, executors, and assigns, hereby:

• Release, waive, and discharge Peaceful Side Brewery, its owners, employees, and affiliates from any liability for injury, illness, loss, or damages arising from participation in the tour.
• Assume full responsibility for any risks, whether known or unknown, associated with the tour.
• Agree to indemnify and hold harmless the brewery from any claims, costs, or expenses resulting from my participation.

Consent to Medical Treatment

In the event of an injury or medical emergency during the tour, I consent to receive medical treatment deemed necessary. I understand I am responsible for any associated costs. ____________________________________________________________________________________

Alcohol Consumption Acknowledgment

I affirm that I am at least 21 years of age and understand that proof of age may be required. I accept full responsibility for my alcohol consumption during the tour.
